Youth Health Talk

YouthHealthTalk
Youthhealthtalk enables young people, their family and friends, and professionals such as doctors and teachers to understand young people's experiences of health, illness and life in general. The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

New Patient Registration

We welcome new patients who live within our practice area. The reception team will advise new patients if their home is within our area and patients can also check this on the Policies section of the website - Our Practice Area. All new patients are asked to complete a registration form, medical and alcohol consumption questionnaires and sharing consent form prior to registration at the practice. These are available from our receptionists and can also be downloaded from this page. The practice requests patients wishing to register with us bring along a means of identification, ideally with a photograph on it. This also enables us to provide new patients with access to our online services for appointment booking and requesting repeat medication - please complete the form below.

We are only able to accept a registration on receipt of the fully completed registration form, medical and alcohol consumption questionnaires and sharing consent form. Incomplete forms will be returned to patients requesting to register. Please also bring the completed online services form and your ID.

New patients can see a GP or Nurse on the day following processing of their complete registration forms.

Patients on repeat medication will need to see a doctor before getting their first prescription. An appointment will be offered as appropriate. We advise patients who wish to register with us to obtain one month's supply of repeat medication from their current GP before registering with Brig Royd Surgery to ensure a smooth handover. 

All new patients are welcome to make an appointment within six months of registration.

All patients are also able to apply for online access to their medical record (once they are fully registered at the practice) - click "Online Services" from the right hand tab.

We also refer new patients to our Privacy Notice, which details the information we hold about patients, how we use it and the legal basis under which it is processed. This can be found under Policies - GDPR and Confidentiality.
