See your GP Health Record in the NHS App!

Having access to your health record allows you to:
- Feel more involved in your healthcare decisions
- Check details before an appointment
- Keep track of test results
- Share information easily if you are receiving care elsewhere
Keeping your information safe
The NHS App is secure and protected. Only you can see your record once you’ve verified your identity. If you use a shared device, make sure you log out after each use.
Sharing Your Medical Record
Sometimes your medical information needs to be shared between different NHS services – for example, between your GP practice and the District Nursing team. This helps healthcare professionals access the most up-to-date information so they can give you the best possible care.
Your consent matters
We will always ask for your permission before your medical information is shared. You decide:
- What information can be shared (for example, from your GP record).
- Which services can see it (for example, district nurses, but not podiatry).
How it works
For instance, district nurses may need to see your GP record to support your care, but the podiatry service may not. In this case, you can agree for your GP practice to share information with district nurses, while choosing not to share it with podiatry.
This means you stay in control of your information, and it is only shared on a “need to know” basis.
