Annual Review Appointments for Patients with Chronic Diseases

The majority of annual reviews for patients with a chronic disease are undertaken by our Advanced Nurse Practitioner, our Practice Nurses, and not our GPs. The length of appointment and the timing of these appointments varies with the review required. Please do not try to book Annual Review appointments online. Please telephone the surgery to make these appointments. Our staff are trained to book the length of appointment/s required and to advise on any requirements from you before or at the appointment. Please note that invitations to attend for Annual Reviews are generally triggered by the requirement for a repeat prescription of your medication so please ring to book your Annual Review as soon as you receive the invitation.
